Genomics-based tests to measure the probability of metastasis of breast cancer are becoming more popular in the prognostic testing market. M.D. Anderson Cancer Center, Agendia and Exagen are coming up with similar discoveries to either compete with or complement Genomic Health's Oncotype DX. Marketed since 2004, Oncotype DX has pioneered personalized medicine. It can calculate recurrence of the breast cancer disease among node-negative/estrogen receptor-positive early-stage patients. Using RT-PCR, recurrence rate is derived from the expression level of 21 genes. The test is covered by Medicare and provides reluctant patients a gauge in deciding whether to undergo chemotherapy. It is being used in a clinical trial to fine-tune treatment for a subgroup. M.D. Anderson Cancer Center researchers have developed a prognostic assay. It quantifies gene expression, predicting sensitivity to chemotherapy. The test is useful in classifying patients, thus, identifying appropriate therapies for them. Exagen's tests predict breast cancer metastasis by identifying patients with invasive ductal carcinoma. Patterns of genomic amplification (PGA) in three genes are used as indicators of recurrence. Agendia's proprietary process involves analysis of genes in fresh tissues run on an Agilent microarray platform. The success of genomics-based tests will set a new standard of care for breast cancer.
